Outpatient Diagnostic Errors: Unrecognized Hyperglycemia

ثبت نشده
چکیده

CASE DEFINITIONS. We identified all patients with abnormal test results suggestive of diabetes (i.e., hemoglobin [Hb] A1c ≥ 7.0% or plasma glucose ≥ 200 mg/dL) on one or more occasions. Patients were considered to have recognized diabetes if they had an ICD-9 diagnostic code for diabetes in the administrative database or a diagnosis mentioned in their medical record (on the basis of medical record review of a random sample of 30% of patients with abnormal test results and no ICD-9 code). Patients with unrecognized diabetes did not have an ICD-9 code or a medical record diagnosis.

Association between hyperglycemia at admission during hospitalization for acute myocardial infarction and subsequent diabetes: insights from the veterans administration cardiac care follow-up clinical study.

OBJECTIVE Among patients with acute myocardial infarction (AMI) without known diabetes, hyperglycemia at admission is common and associated with worse outcomes. It may represent developing diabetes, but this association is unclear. Therefore, we examined the association between hyperglycemia (≥140 mg/dL) at admission and evidence of diabetes among patients with AMI without known diabetes within...

The frequency of diagnostic errors in outpatient care: estimations from three large observational studies involving US adult populations

BACKGROUND The frequency of outpatient diagnostic errors is challenging to determine due to varying error definitions and the need to review data across multiple providers and care settings over time. We estimated the frequency of diagnostic errors in the US adult population by synthesising data from three previous studies of clinic-based populations that used conceptually similar definitions o...

Persistent outpatient hyperglycemia is independently associated with decreased survival after primary resection of malignant brain astrocytomas.

OBJECTIVE Patients with malignant brain astrocytomas are at high risk for developing hyperglycemia secondary to frequent corticosteroid administration. Several clinical studies have shown that hyperglycemia is associated with poor outcome in multiple disease states. Furthermore, hyperglycemia augments in vitro astrocytoma growth, whereas hypoglycemia attenuates in vitro astrocytoma cell growth....

Application of an Automated Surveillance System to Evaluate Outpatient Care of Pneumonia

Introduction With economic pressures to shift the care of community-acquired pneumonia (CAP) to the ambulatory setting, there is a need to ensure safety of outpatients with CAP. The use of claims data alone remains the primary strategy for identifying these patients, but billing information often does not match the clinical diagnosis and does not have the ability to find unrecognized cases. In ...
